HCT Avoidable Day Tracking
An “Avoidable Hospital Day” (AHD) is defined as a patient day in the hospital during which acute hospital services were not rendered or could have been safely and appropriately performed in an alternate setting. The HCT Avoidable Day Tracking Form delineates reason (Facility Causes, Medical Staff/Physician Causes, Case Management Causes, Patient/Family, Discharge Planning Causes), case manager, and date of each avoidable day. This document helps qualify avoidable days to ensure that hospitals can identify the source which causes the highest number of avoidable days, typically a large financial impact for a facility.
